Physicals/Immunizations
Parents of Pre-School, Kindergarten, and New Students, please remember we need proof of immunizations and a recent physical on file before the first day of school. Students will be excluded from school without these. Please refer to page 21 of the Parent/Student Handbook for additional information.

This registration is for ALL NEW and RETURNING families to the district.
All students must be registered through the online process. Click on the Skyward Link found on this page to begin the registration process.

Health Requirements

All students, both returning and new to the district, must have the following on file with the District by the first day of the 2022-2023 school year. Documents will be accepted online or through email to the school.
- Birth certificate
- Up-to-date immunizations
- Illinois physical (required of new students, pre-school students, kindergarten students, and 6th grade students)
- Parental agreement (if applicable)
